Obtain/verify contact information: corporate or billing address if different from location, the Name of manager/owner/scheduler, EMAIL ADDRESS, Best contact Phone Number, Fax number if available. Note all information in a comment on the location. Semi-Annual Kitchen Suppression System Inspection as per NFPA 17A. Fire Alarm system (if present) must be placed on test prior to performing service. Access to FACP for purpose of reset must be made available prior to conducting full function test. If unable to disarm alarm or place on test document same on inspection report as reason why full function test did not occur. Attempt to obtain Extinguisher Services, if approved add comment for month to start service so office can set up recurring services.
